QUESTION: How are bio-identical hormones usually taken?

DR. SANCHEZ: For estrogen replacement and testosterone replacement oral pills don’t work very well; acid in the mouth and stomach interfere with delivery of the medications and diminish their effectiveness. What we’re aiming at is the correct level of hormones in the blood, not the gut. Restoring youthful vigor and functioning requires maintaining accurate hormone levels for long periods of time.

To achieve those accurate levels we usually start patients with topically applied cream or a trans-dermal patch. The cream is applied daily and a measured amount is rubbed into the skin. Some patients don’t respond well to the patches, they get a rash, or their skin won’t readily absorb the creams. We can provide injectable doses for these patients, and, we teach them to inject themselves daily or weekly.

But I hate needles!

DR. SANCHEZ: Many people do! And a longer-lasting and more uniform hormone dosage is desirable as well. Hormone Pellet Therapy is a better delivery method than creams, patches or injections. Once a proper dosage has been determined we implant hormone pellets in the patient’s gluteus maximus muscles. It is a quick outpatient procedure and then your hormone levels are maintained 24/7 for several months.

The bioidentical hormone pellets we use are well-proven, and they deliver bio-identical hormones in a measured way directly into the bloodstream. Your hormone system is quite complex and we must be careful with any changes. In my opinion pellet therapy is the best way to maintain hormone replacement levels accurately.

As we’ve discussed several important things are controlled by our hormone levels: brain function, our moods, sleep and, of course, sexual functioning that most patients focus on. To enjoy our lives to the fullest all these things should be brought  to optimum levels. Our old age does not have to be a withdrawal from interests and activities we love. Gradual decline is not inevitable – bio-identical hormones can slow or even reverse so-called “normal” decline as we age.
